With any sex device, especially improvised ones, care must be taken to ensure no damage will be done the area being stimulated.  Also, inserted items without a base and those with a base that are inserted too far are especially dangerous as the suction within the body can cause items to become trapped and require medical assistance to remove.

Also, from purely anecdotal references, it is generally considered a bad idea to insert the penis into a vacuum cleaner in an attempt to gain sexual gratification.  Not only does the point where a vacuum hose attach to the body of the device generally have moving parts behind it (supposedly people have actually had their penises painfully mangled or removed by these moving parts), but the extreme suction can cause damage to the penis.

This can also include improvised devices, including a huge variety of things that both men and women can rub up against (blankets, pillows, walls, beds, washing machines, couches, counter tops, a carpeted floor, literally anything that provides adequate friction can and has been used to get off on), items that the penis can be inserted into (once again, if it provides friction and doesn't do damage to the flesh, it's been used), and items that can be inserted into the vagina or anus (basically, if it's remotely phallic, it's been used to great success, though it is often claimed that vegetables are a favorite for this).
